Reimbursement
As a private, cash-based occupational therapy practice, we do not accept insurance. However, we strive to make the reimbursement process as smooth as possible for you. Upon request, we will provide a superbill that you can submit to your insurance provider for potential reimbursement. We recommend reaching out to your insurance company directly to understand your out-of-network benefits and reimbursement eligibility before scheduling your Balanced Living OT appointment.
1. Payment To Us
Payment for services is required upfront, based on the rates we have outlined on our website or provided during scheduling.
2. Superbill For Insurance Submission
After each session and if requested, we will provide you with a superbill. This document includes all of the information for your insurance company in order to process a claim such as: diagnosis codes, service details, and provider credentials.
3. Reimbursement From Insurance
You can submit the superbill to your insurance company for potential reimbursement. If your insurance plan includes out-of-network benefits, your insurance company may reimburse you directly for a portion of the cost.
4. No Impact On Our Fees
Your reimbursement does not affect the payment you made to us. Once payment is made to Balanced Living OT, it is your responsibility to work with your insurance provider to recover eligible funds.
