We service the Capital Region of New York. All services are provided by Board-Certified Wound, Ostomy, and Continence Nurses (CWOCN).

Our wound and ostomy care services support adults with chronic wounds, surgical wounds, pressure injuries, ostomies or challenges managing skin integrity around stomas. We also help caregivers and family members who assist with wound or ostomy care at home.

Who is eligible for WOC services?

If you’re dealing with non‑healing wounds, frequent wound complications, persistent irritation around your stoma, difficulty managing an ostomy pouch, or if you’re unsure how best to care for wounds or stomas at home.

Our Services

In Person Initial Evaluation

Comprehensive history and client intake (including prior wound/ostomy history, medications, nutrition, lifestyle, and caregiver involvement).

  • Full skin and wound/ostomy assessment:

    • Location, size, and depth of wound(s)

    • Wound bed tissue description, exudate, and odor

    • Surrounding skin integrity (erythema, maceration, induration)

    • Risk assessment for pressure injuries using validated scales

    • Ostomy stoma assessment (size, color, protrusion/retraction, peristomal skin)

  • Physiological measures

    • Blood pressure (BP)

    • Heart rate (HR)

    • Oxygen saturation (SpO₂)

    • Temperature (if indicated, for infection screening)

  • Functional environment review: bed, chair, bathroom, and support surfaces to identify risks for wounds and ostomy complications.

  • Immediate care plan

    • Recommendations for wound dressing changes and ostomy care tailored to patient condition.

    • Pressure offloading strategies and positioning.

    • Nutrition and hydration guidance related to skin integrity.

    • Red flag education for when to seek higher-level medical care.

  • Follow-up: 10-minute Telehealth phone call to review progress, clarify instructions, and reinforce recommendations.

Telehealth Initial Evaluation

  • Review of medical history, ostomy/wound history, and current challenges.

  • Visual assessment via secure video platform.

    • 1. Wound or stoma inspection (size, shape, skin condition).

    • 2. Dressing/ostomy appliance review (fit, leakage, wear time).

  • Physiological review based on client self-reports (BP, HR, SpO₂, if home monitor is available).

  • Education on wound/ostomy care, skin protection, and complication prevention.

  • Written and verbal care instructions.

  • Follow-up: 10-minute Telehealth phone call to review progress, clarify instructions, and reinforce recommendations.

Telehealth consultations available within 12 hours of booking. In-person consultations are scheduled as soon as possible. Ostomy re-education includes three pouch changes and personalized home evaluation for pressure injury prevention. All services performed by a licensed, board-certified WOC nurse.

Ostomy Re-Education Package

  • Step-by-step teaching to client and/or caregiver during each change.

  • Full stoma and peristomal skin inspection at each visit.

  • Pressure injury risk check with each encounter.

  • Appliance fit assessment and troubleshooting of leaks. odor, or skin irritation.

  • Documentation and tracking of stoma size, skin changes, and pouch wear time.

  • Education on nutrition, hydration, and lifestyle adaptations for ostomy management.

  • Written summary with tailored recommendations after package completion.

  • If a full evaluation is needed, the call fee can be applied toward the cost of an evaluation.
